Registration for Kindergarten: Next School Year


Registering your child for the 2019-20 school year is a 5-step process:

  1. Gather required documentation
  2. Complete online registration form
  3. Schedule registration appointment using SignUp Genius
  4. Attend registration appointment or wait to be called for your phone appointment
  5. Attend screening appointment with your child in August

STEP 1: Gather required documentation

Requirements for Enrollment

You will need to bring the required documentation with you to your registration appointment or upload it with your online registration form. Note: If you already have a student in the District and upload your documentation with the online registration form, you will have the option to sign up for a phone appointment instead of having to make a trip to the Welcome Center. VERY IMPORTANT! There is a Language Usage Survey attached to the bottom of the online registration form that must be completed and uploaded if you are scheduling a phone appointment. Under “Documents” at the bottom of the online registration form locate the document titled “Language Usage Survey”. Download the form, complete it, and upload it with the rest of your documents.

To register in the Hilliard City School District it is the parent’s/legal guardian’s responsibility to provide the following documents:

1. PROOF OF RESIDENCY – Two are Required

Choose your first document from the A list and the second document must be provided from the B list:

  • Up-to-Date Rental/Lease Agreement with custodial parent’s name (and ALL adult residents) listed and signed by the landlord/rental agent; or
    If on month-to-month lease, please obtain a Statement of Occupancy from the Welcome Center for the landlord to complete and sign, listing everyone living at the residence. (This should be provided in addition to a copy of the signed lease.) All signed rental contracts must accompany the attached HCSD Residency Verification Release.
  • Current Mortgage Statement with custodial parent’s name listed
  • Purchase/Contract Agreement to buy/lease property (if within 90 days of enrollment)
  • Official Confirmation of address change filed with the US Postal Service mailed to your new address
  • Current (within 30 days) Utility Bill, in custodial parent’s name showing service address at Hilliard Schools’ residence
  • Current (within 30 days) Government Mailing (i.e., child support, government assistance)

*If your mortgage statement or utility bills are processed through a type of online payment, it is your responsibility to obtain current statements, as required.

2. BIRTH CERTIFICATE

The original or a certified copy of your child’s birth certificate. (If you do not have your child’s birth certificate, contact the Bureau of Vital Statistics in the city your child was born to request another copy. This process can take up to three weeks to receive this birth certificate.)

3. OFFICIAL IMMUNIZATION RECORD:

If you have an incomplete record or you do not have one, please contact your physician’s office for a copy of this record. You MUST bring this when registering your student, even if it is not complete.

Required immunizations

Diphtheria, Tetanus, Pertussis (DTP)

  • Kindergarten: Four (4) or more doses of DTaP or DT, or any combination. If all four doses were given before the 4th birthday, a fifth (5) dose is required. If the fourth dose was administered at least six months after the third dose, and on or after the 4th birthday, a fifth (5) dose is not required.
  • Grades 1-12: Four (4) or more doses of DTaP or DT, or any combination. Three doses of Td or a combination of Td and Tdap is the minimum acceptable for children age seven (7) and up.
  • Grades 7-12: One (1) dose of Tdap vaccine must be administered prior to entry.

Polio

  • K-8: Three (3) or more doses of IPV. The FINAL dose must be administered on or after the 4th birthday regardless of the number of previous doses. If a combination of OPV and IPV was received, four (4) doses of either vaccine are required.
  • Grades 9-12: Three (3) or more doses of IPV or OPV. If the third dose of either series was received prior to the fourth birthday, a fourth (4) dose is required; If a combination of OPV and IPV was received, four (4) doses of either vaccine are required.

Measles, Mumps, Rubella (MMR)

  • K-12: Two (2) doses of MMR. Dose one (1) must be administered on or after the first birthday. The second dose must be administered at least 28 days after dose one (1).

Hepatitis B

  • K-12: Three (3) doses of Hepatitis B. The second dose must be administered at least 28 days after the first dose. The third dose must be given at least 16 weeks after the first dose and at least 8 weeks after the second dose. The last dose in the series (third or fourth dose), must not be administered before age 24 weeks.

Varicella (Chickenpox)

  • K-8: Two (2) doses of varicella vaccine must be administered prior to entry. Dose one (1) must be administered on or after the first birthday. The second dose should be administered at least three (3) months after dose one (1); however, if the second dose is administered at least 28 days after the first dose, it is considered valid.
  • Grades 9-12: One (1) dose of varicella vaccine must be administered on or after the first birthday.

MCV4 (Meningococcal)

  • Grades 7-9: One (1) dose of meningococcal (serogroup A, C, W, and Y) vaccine must be administered prior to entry.
  • Grades 12: Two (2) doses of meningococcal (serogroup A, C, W, and Y) vaccine must be administered prior to entry.

TB Testing

  • Hilliard Schools requires any student born outside of the United States to have a tuberculin skin test. Hilliard Schools also requires any student who has spent one month (32 days) or more days in a high-risk TB country to have a tuberculin skin test. Enrollment for new students may be deferred until documentation of a negative TB test or negative chest x-ray is received by the registration compliance officer. Re-entry for students who have traveled outside the United States for more than one month (32 days) may be deferred until such documentation is received by school personnel.

4. CUSTODY PAPERS

(If applicable) Per State of Ohio Law, (ORC 3313.672) and the Missing Children’s Act, we are required to have an official, court stamped custody document when both natural parents are not in the home.

STEP 3: Schedule registration appointment using SignUp Genius

This is required for both walk-in and phone appointments.

Click the name of your “home” Elementary School to schedule a registration appointment. These links will take you to the kindergarten registration schedule on a site called SignUp Genius. Contact the Hilliard City Schools Records Compliance Office at 614-921-7096 if you are unsure which elementary building is the “home” school for your child.

STEP 4: Attend registration appointment or wait to be called for your phone appointment

You will need to complete the online registration form before your scheduled appointment. If you did not qualify for a phone appointment, please be sure to bring all required documentation if you did not upload it with the online registration. You will go to the Welcome Center at 2140 Atlas Street, Columbus or wait to be called at your appointed time. When all registration documents have been received and your online registration form has been reviewed, you will be able to schedule a time to bring your child for kindergarten screening.

STEP 5: Parent and child attend screening day in August

Screening will take place at the school assigned for your child. Students will not be screened until all required paperwork is submitted.

How do I schedule a registration appointment?

What do I need to bring to the registration appointment?
Please make sure you completed your online registration form before your scheduled appointment. All required documents will need to be uploaded with your registration form or brought to the meeting. These documents include, child’s birth certificate, official immunization record, parent proof of residency, and court date stamped proof of custody (if applicable) or proof of “pending” custody. If you are unsure about what to bring, please call our Records Compliance Officer at 614-921-7096. Only completed packets with all required documents will be processed. (Children do not need to attend)

How do I schedule an appointment for screening?
When all necessary registration documents have been received, you will then make an appointment for kindergarten screening on your “home” school’s assigned screening day. Students will not be screened until all required paperwork is submitted. Screening dates are time sensitive, therefore parents should submit all registration documents on their identified registration day.

What should I expect during screening?
This is an exciting time for students, families and staff. You will tour the building and meet some of the school personnel. In addition, district-trained kindergarten screeners will test your child’s readiness skill. This is not a pass/fail screening. Your child’s vision, speech, and hearing may also be evaluated. You can expect this process to take approximately 60-90 minutes. You will receive more detailed information from your child’s home building after your registration is complete.

What if I missed my registration day in February?
Fill out the online registration form or visit our Student Welcome Center located at 2140 Atlas Street, Columbus, OH. You will then need to bring in all of your registration documentation to the Record Compliance Officer by calling 614-921-7096 to schedule an appointment.

When and where can I apply for an intra-district building transfer?
Transfer Request forms will be available online January 28 – March 1, 2019 and they will also be available in all school buildings and at the District’s Central Office.

Registration for kindergarten will not be complete, and students will not be added to class rosters, until all items are submitted and approved by the District Records Compliance Officer.
