Registering your child for the 2021-22 school year is a 5-step process:
Requirements for Enrollment
You will need to bring the required documentation with you to your registration appointment or upload it with your online registration form. Note: If you are not able to upload all of your documentation, you may also email it to welcomecenter@hboe.org.
To register in the Hilliard City School District it is the parent’s/legal guardian’s responsibility to provide the following documents:
1. LANGUAGE SURVEY – The State requires this document for ALL students
2. PROOF OF RESIDENCY – Two are Required
Choose your first document from the A list and the second document must be provided from the B list:

*If your mortgage statement or utility bills are processed through a type of online payment, it is your responsibility to obtain current statements, as required.
*If you share a home or apartment with another family, please contact the Welcome Center at (614) 921-7096.
3. BIRTH CERTIFICATE
The original or a certified copy of your child’s birth certificate. (If you do not have your child’s birth certificate, contact the Bureau of Vital Statistics in the city your child was born to request another copy. This process can take up to three weeks to receive this birth certificate.)
4. OFFICIAL IMMUNIZATION RECORD:
If you have an incomplete record or you do not have one, please contact your physician’s office for a copy of this record. You MUST bring this when registering your student, even if it is not complete.

Required immunizations
DTaP/DT Tdap/Td
Diphtheria, Tetanus, Pertussis
Kindergarten
Four (4) or more doses of DTaP or DT, or any combination. If all four doses were given before the fourth birthday, a fifth (5th) dose is required. If the fourth dose was administered at least six months after the third dose, and on or after the fourth birthday, a fifth dose is not required.
Grades 1-12
Four (4) or more doses of DTaP or DT, or any combination. Three doses of Td or a combination of Td and Tdap is the minimum acceptable for children ages 7 and older, with proper spacing.
Grades 7-12
One (1) dose of Tdap vaccine must be administered on or after the 10th birthday.
POLIO
K-10
Three (3) or more doses of IPV. The FINAL dose must be administered on or after the fourth birthday regardless of the number of previous doses. If a combination of OPV and IPV was received, four (4) doses of either vaccine are required.
Grades 11-12
Three (3) or more doses of IPV or OPV. If the third dose of either series was received prior to the fourth birthday, a fourth (4th) dose is required; if a combination of OPV and IPV was received, four (4) doses of either vaccine are required.
MMR
Measles, Mumps, Rubella
K-12
Two (2) doses of MMR. Dose one (1) must be administered on or after the first birthday. The second (2nd) dose must be administered at least 28 days after dose one (1).
HEP B
Hepatitis B
K-12
Three (3) or (4) doses of hepatitis B. The second dose must be administered at least 28 days after the first dose. The third dose must be given at least 16 weeks after the first dose and at least eight weeks after the second dose. The last dose in the series (third or fourth dose) must not be administered before age 24 weeks.
VARICELLA
(Chickenpox)
K-10
Two (2) doses of varicella vaccine must be administered prior to entry. Dose one (1) must be administered on or after the first birthday. The second dose should be administered at least three months after dose one (1); however, if the second dose is administered at least 28 days after the first dose, it is considered valid.
Grades 11-12
One (1) dose of varicella vaccine must be administered on or after the first birthday.
MCV4
Meningococcal
Grades 7-11
One (1) dose of meningococcal (serogroup A, C, W, and Y) vaccine must be administered prior to entry.
Grade 12
Two (2) doses of meningococcal (serogroup A, C, W, and Y) vaccine must be administered prior to entry.
TB Testing
Hilliard Schools requires any student born outside of the United States to have a tuberculin screening. Hilliard Schools also requires any student who has spent one month (32 days) or more days in a high-risk TB country to have a tuberculin screening. Enrollment for new students may be deferred until documentation of a negative TB screening is received by the registration compliance officer. Re-entry for students who have traveled outside the United States for more than one month (32 days) may be deferred until such documentation is received by school personnel.
5. CUSTODY PAPERS
(If applicable) Per State of Ohio Law, (ORC 3313.672) and the Missing Children’s Act, we are required to have an official, court stamped custody document when both natural parents are not in the home.

You will need to complete the online registration form before your scheduled appointment. If you did not qualify for a phone appointment, please be sure to bring all required documentation with you to your appointment. You will go to the Welcome Center at 2140 Atlas Street, Columbus, or wait to be called at your appointed time. When all registration documents have been received and your online registration form has been reviewed, you will be able to schedule a time to bring your child for kindergarten screening.

How do I schedule a registration appointment?

What do I need to bring to the registration appointment?
Please make sure you completed your online registration form before your scheduled appointment. All required documents will need to be uploaded with your registration form or brought to the meeting. These documents include language survey, child’s birth certificate, official immunization record, parent proof of residency, and court date-stamped proof of custody (if applicable) or proof of “pending” custody. If you are unsure about what to bring, please call our Records Compliance Officer at 614-921-7096. Only completed registrations with all required documents will be processed. (Children do not need to attend)
How do I schedule an appointment for a screening?
At your registration appointment, if all necessary registration documents have been received, you will make an appointment for kindergarten screening on your “home” school’s assigned screening day. Students will not be screened until all required paperwork is submitted. Screening dates are time-sensitive, therefore parents should submit all registration documents on their identified registration day.
What should I expect during screening?
This is an exciting time for students, families, and staff. You will tour the building and meet some of the school personnel. In addition, district-trained kindergarten screeners will test your child’s readiness skills. This is not a pass/fail screening. Your child’s vision, speech, and hearing may also be evaluated. You can expect this process to take approximately 60-90 minutes. You will receive more detailed information from your child’s home building after your registration is complete.
What if I missed my registration day in January/February?
Fill out the online registration form and upload your documentation. Call 614-921-7096 to schedule an appointment, after completing the online registration, if you are not able to upload your documentation.
When and where can I apply for an intra-district building transfer?
Transfer Request forms will be available online from January 25 – February 26, 2021, and they will also be available in all school buildings and at the District’s Central Office.
Transfer forms must be completed and returned to Mrs. Vicky Clark, Assistant Superintendent, 2140 Atlas Street, Columbus, OH. 43228.

Registration for kindergarten will not be complete, and students will not be added to class rosters until all items are submitted and approved by the District Records Compliance Officer.
