2. If you do not have an account, you will need to create one
3. Login with your account
4. Scroll to bottom of page
5. Click on HCS Transcript/Records Request
6. Check box under PAY
7. Click Continue Payment
8. Fill out Form
9. Fill out Payment information

2. Fill out, Print and Mail the form with a non-refundable payment to
