How Do I Prepare for the Transition Out of High School?

During your child’s educational journey, your child will experience many transitions. Starting with the transition from home to preschool and ending with the transition out of high school, each transition has its challenges and opportunities.

Planning your child’s transition from high school to adulthood is one of the key steps in preparing them for a successful future. 

The Special Education Secondary Transition Roadmap, developed by the Ohio Department of Education & Workforce, may help you with these transitions.

Transition Specialists

Each high school has a Transition Specialist who works with students and families to help coordinate a transition plan that aligns with the student’s goals for post secondary education, employment, and independent living/community integration. 

Many post secondary programs and county agencies require applications and may have waiting lists.  Connecting with your child’s Transition Specialist will help your child reach their post secondary goals in a timely manner.

Resources to Assist Families Navigating the Transition from High School to Adulthood

Differences Between High School and College Accommodations for Students with Disabilities 

Franklin County Board of Developmental Disabilities Services (FCBDD)

FCBDD is a county agency that supports children and adults with certain developmental disabilities. FCBDD services may include home support, transportation, future housing planning, employment services, and funding for adult day programs to promote independence and community involvement. Click on the “Getting Started” tab at the top of the home page for Eligibility requirements and other information.

Opportunities for Ohioans with Disabilities (OOD) – Student Services  

OOD provides vocational rehabilitation services to eligible individuals with disabilities to assist them in attaining and maintaining competitive integrated employment. OOD works with adults seeking to enter the workforce or retain a job, and with youth beginning at age 14 to assist them as they transition from high school into college and/or the workplace.


National Center for College Students with Disabilities (including career and technical education)

The NCCSD conducts research and provides information and resources about disability and higher education or attending postsecondary education, which includes Career and Technical Education (CTE) programs.

Think College

Think College provides resources, technical assistance and training related to college options for students with intellectual disability, and manages the only national listing of college programs for students with intellectual disability in the United States. To make college accessible and achievable for individuals with intellectual disabilities, leading to greater independence and fulfilling lives. 

Tolles Career & Technical Center

Tolles Technical and Career Center provides career technical education for students in high school. Featuring Sophomore Career Academy, Junior and Senior Programming, Tolles offers a variety of opportunities and internships for students interested in career tech pathways.
