Parent/Guardian Book Selection Exemption Form

If you wish to request that a book or resource not be checked out to your student from their Innovation & Discovery/Media Center, you must complete the form below. Each submitted form will accept up to 10 book titles/author’s names. Please go to the Destiny Discover catalog on your child’s school website under Academics > Innovation Discovery Center OR Media Center to verify that the book/resource currently exists at that location before submitting this form.

Your submission will be shared with your child’s school and added to your child’s information record in Destiny (Library Resource System) only if that item exists at that location. Items submitted through this form will be on your student’s Destiny record for the course of the current school year only. Should you wish to have books/resources exempt for your student next year, you will need to submit this form again for each item.

The Hilliard City School District’s Curriculum Department is in the midst of a multi-year curriculum revision cycle for several content areas. An important part of this process is seeking input from our parents on what is important to them in each content area. The next content area that a district committee will revise is Mathematics. Bradley High School is in the process of applying for our Purple Star School designation from the Ohio Department of Education. The Purple Star Award recognizes schools that show a major commitment to students and families connected to our nation’s military. Schools that earn the award will receive a special Purple Star recognition to display on site to signify our continued commitment to recognition, fundraising and student support services for those families and students who serve or have served our country.

It is IMPORTANT that we capture you or your student’s (both past and present) military branch for future events and recognition.  As a part of these efforts, we have created a Military Resources webpage, a space for our military families to find resources. In order for athletes to participate, all of the following must take place. 

**Physicals must be updated prior to participation.  Upload two forms from the physical — Physical Exam Form and Medical Eligibility Form — to the parent’s Final Forms account (paper physicals will not be accepted).  **Both athlete and a parent/guardian must sign all forms in Final Forms.  **Participation Fees must be paid prior to the first competition through your MyPaymentsPlus account (check or cash will not be accepted).  All athletes, including free or reduced lunch, must complete the payment process through MyPaymentsPlus.  Free or reduced lunch must check out through MyPaymentsPlus by paying half (reduced lunch) or $0 (free lunch) payment.  Fees must be paid prior to the first competition.

ATHLETES MAY NOT PARTICIPATE IN THE COMPETITIVE SEASON UNTIL ALL OF THE ABOVE IS COMPLETED.
