The mandatory Fall Sports Parent/Athlete Meeting will be held on Wednesday, August 7 at 5:30 pm in the Performing Arts Center. This meeting is for all Fall Sports – Grades 7-12.
FINAL FORMS IS HERE!!!
The Bradley & Memorial Athletic Departments are transitioning our student athletic data management to Final Forms. Our goal is to improve our service and communication regarding student data, (i.e. physicals, forms, pay to play, and eligibility.) We need all parents to follow the directions linked below to create and account. There will be follow-up information regarding the rest of the process at the parent meeting on Wednesday, August 7th. We are excited to partner with Final Forms.
Reminder – All Athletes are required to have a valid physical on file prior to tryouts.
PARTICIPANTS MUST HAVE AN UP-TO-DATE PHYSICAL IN ORDER TO PRACTICE. THIS INCLUDES TRYOUTS! PLAYERS WILL NOT BE PERMITTED TO START WITHOUT A CURRENT PHYSICAL. ALL PAPERWORK MUST BE TURNED IN TO THE AD AND/OR COACH BEFORE PRACTICES/TRYOUTS. See the “Parent to do list” before the start of the season.
ONLINE FORMS AND ATHLETIC FEES
PLEASE NOTE THE ATHLETIC PARENT MEETING(s) from the list. These parent meetings are generally held in the Performing Arts Center (PAC) at Hilliard Bradley High School, with DATES and TIMES varying per season.
ATTENTION ATHLETES AND PARENTS: Please make sure your EMERGENCY CONTACT INFORMATION and INSURANCE INFORMATION is updated on Home Access! Just a reminder, all athletes must be covered by medical insurance. Insurance can be purchased through the school if needed. Contact the Athletic Department for details. You also need to electronically “sign-off” on the Risk Acknowledgement, Travel Permit, Concussion and Athletic Handbook Acknowledgement documents, located on Home Access. Please review and sign-off of these documents in Home Access in order to be prepared to participate on the first day of practice/tryouts. Please complete these sign-offs online before the first scrimmage. Home Access Link. After you sign in, follow the update instructions at the top of the page and open the “Medical and Misc. Information” tab to update insurance information and forms sign-off. Make sure to check out the links on the bottom of the Medical Information page to view the OHSAA video, if applicable.

PARTICIPATION FEES: A participation fee of $80 is expected to be paid in full prior to the start of the season. You may pay this fee online at www.mypaymentsplus.com, or by cash or check. If you are paying by cash or check, please make the checks payable to Hilliard City Schools. Your athlete should give the check or cash to the Athletic Office at Bradley or in the Main Office at Memorial. DO NOT give participation fees to coaches. Policy For Tryouts
Our commitment to athletics is ever present and dedicated to providing quality experiences for young student athletes. The length of time for athletic tryouts may vary from sport to sport, but is generally three days. Any athlete trying out for a team is guaranteed an opportunity within those three days to demonstrate their athletic abilities in that sport. Students have the opportunity to attend part or all of the tryout period. Attending all tryouts may enhance your chances, but does not guarantee a position over another athlete who has a better tryout but does not attend all tryout days. The coach’s choice is based solely on a student athlete’s tryout. All coaching decisions are final.
Middle School Eligibility Requirements
Entering 7th graders are automatically eligible for the first nine weeks of school. All other following seasons’ eligibility requirements are based on the previous quarter’s grades.
- 8th graders: Fall athletic eligibility is based on your 4th nine weeks grades from the 7th grade.
- SUMMER SCHOOL DOES NOT MAKE YOU ELIGIBLE FOR FALL SPORTS.
- THE PREVIOUS QUARTER’S GRADES DETERMINE ELIGIBILITY FOR THE CURRENT QUARTER.
- In any quarter: must have a 1.5 or higher to be eligible to participate and must pass 5 subjects!
- 1.5-1.75 GPA places a student athlete on academic probation, and a weekly progress report is necessary to maintain your athletic eligibility.
- Missing any assignments in your classes will cause you to miss practices and/or contests, and may result in reduced playing time.

*Every athlete must turn in a completed OHSAA Physical Form PRIOR TO practice or tryouts. It is the Pre-Participation Form on the OHSAA site, and should be fully completed. The updated form can be downloaded here:
