Registration for Kindergarten: Next School Year

Registering your child for the 2012-13 school year is a 3 step-process:

  1. Attend Pre-Registration night and fill out online form
  2. Attend registration appointment
  3. Attend screening appointment for you and your child

STEP 1: Attend pre-registration night and fill out online form

Pre-Registration will be held at the child’s “home” school building based on residency and current school district boundaries. Contact the Hilliard City Schools Administrative Office at 921-7000 if you are unsure of which elementary building is the “home” school for your child.

Parents should go to their "home" school gymnasium to receive registration instructions and to make an appointment for registration.

It is necessary to attend this meeting in order to schedule a time for registration. If you miss the pre-registration, you will need to call your home school to make an appointment. Only one parent is needed at the pre-registration evening and it is not necessary to bring in any paperwork at that time. Children do not need to attend this phase of the registration process. At this time, you will receive information that will detail what is needed in the next phase of the registration process.

STEP 2: Complete online form and then bring appropriate documentation to registration appointment

At pre-registration night, you will be assigned a kindergarten registration date and time. You will need to complete the online registration form before your scheduled appointment. On the day of your registration appointment, one of the custodial parents or guardians must bring the required documentation to the Central Office Annex (5323 Cemetery RD., located behind the main Central Office building). When all necessary registration documents have been received, you will then make an appointment for kindergarten screening on your school’s assigned day.

STEP 3: Parent and child attend screening day

Screening will take place at our Support Services Facility (SSF), located at 2140 Atlas St., Columbus, OH.  View Map    

Students will not be screened until all required paperwork is submitted. On the screening day, your child will be escorted to an area where vision, speech and school-readiness assessments are done. Please plan to spend about one hour completing the screening process.

What do I need to bring to the registration appointment?
Please make sure you completed your online registration form before your scheduled appointment. Bring all appropriate documents to the meeting. We will need to copy the child’s birth certificate, official immunization record, parent proof of residency, proof of custody (if applicable) or proof of “pending” custody. If you are unsure about what to bring, please call our Records Compliance Officer at 921-7096. Only completed packets with all required documents will be processed. (Children should not attend)

How do I schedule an appointment for screening?
When all necessary registration documents have been received, you will then make an appointment for kindergarten screening on your school's assigned screening day. Students will not be screened until all required paperwork is submitted. Screening dates are time sensitive, therefore parents should submit all registration documents on their identified registration day.

What should I expect during screening?
District-trained kindergarten screeners will screen your child. This is not a pass/fail screening. A school nurse will evaluate general health: vision, speech and any learning-related function will also be evaluated. Your child will spend a short time listening to a story, playing and interacting with other children. You can expect this process to take approximately 60 minutes.

What if I did not attend the January registration night and missed my registration day in February?
Fill out the online registration form or visit our New Student Welcome Center located in the Central Office Annex, 5323 Cemetery Rd, Hilliard, OH. You will then need to bring in all of your registration documentation to the Record Compliance Officer anytime between the hours or 8am-12:30pm or 2-4pm.

When and where can I apply for an intra-district building transfer?
In the spring, Elementary Transfer Request forms will be available in all school buildings and at the District's Central Office. They must be received by the end of the business day at a date to be determined.

Registration for kindergarten will not be complete, and students will not be added to class rosters, until all items are submitted and approved by the District Records Compliance Officer.

Requirements for Enrollment

As you prepare to register your student, it is important to be aware of the procedure the District has established so that the process can go smoothly. To register in the Hilliard City School District it is the parent's/legal guardian's responsibility to provide the following documents:

1. PROOF OF RESIDENCY

Proof of residency may be either a current monthly mortgage statement or declaration page from a current homeowner’s insurance policy if you own your home. If you are renting, proof of residency may be a lease agreement, signed by the landlord/agent, that lists the parent/guardian’s name and the name of the students to be enrolled in our district. In situations where one family is sharing a home or apartment with another, please contact the Records Compliance Officer at (614) 921-7096.

2. BIRTH CERTIFICATE

The original or a certified copy of your child’s birth certificate.(If you do not have your child's birth certificate, contact the Bureau of Vital Statistics in the city your child was born to request another copy. It can take up to three weeks to receive this birth certificate. Please plan accordingly.)

3. OFFICIAL IMMUNIZATION RECORD:

If you have an incomplete record or you do not have one, please contact your physician's office for a copy of this record. You MUST bring this when registering your student even if it is not complete.

Required immunizations

Diphtheria, Tetanus, Pertussis (DTP)

  • Preschool: 4 doses of DTaP, DTP, or DT or any combination.
  • Kindergarten: 5 doses of DTaP, DTP, or DT, or any combination, if the fourth dose was administered prior to the 4th birthday
  • Grades 1-12 : 3-4 doses of DTaP, DTP, DT or Td or any combination.
  • Grade 7: 1 dose of Tdap or Td vaccine must be administered prior to entry.
Polio
  • Preschool: 3 doses of OPV or IPV or any combination of OPV or IPV.
  • Kindergarten : 4 doses of any combination of OPV or IPV, the final dose must be administered on or after the 4th birthday regardless of the number of previous doses.
  • Grades 1-12: 4 doses if a combination of OPV or IPV was administered.
Measles, Mumps, Rubella (MMR)
  • Preschool: 1 dose of MMR administered on or after the first birthday
  • K-12: 2 doses of MMR. Dose 1 must be administered on or after the first birthday. The second dose must be administered at least 28 days after dose 1.
Haemophilus Influenzae Type b (HIB )
  • Preschool: 3 or 4 doses depending on the vaccine type, the age when the child began the 1st dose and the last dose must be after 12 months or 1 dose if given on or after 15 months of age
Hepatitis B
  • Preschool: 3 doses of Hepatitis B
  • K-11: 3 doses of Hepatitis B. The second dose must be administered at least 28 days after the first dose. The third dose must be given at least 16 weeks after the first dose and at least 8 weeks after the second dose. The last dose in the series (third or fourth dose), must not be administered before age 24 weeks.
Varicella (Chickenpox)
  • Kindergarten: 2 doses of varicella vaccine must be administered prior to entry. Dose 1 must be administered on or after the first birthday. The second dose must be administered at least 28 days after dose 1.
  • Grade 1-4: 1 dose of varicella vaccine must be administered on or after the first birthday
TB Testing
  • Students born in another country or any student who has traveled out of the country for more than 90 days are required to have a Tuberculin skin test. The test that must be used in the U.S. is the Mantoux Test 5 TU PPD. Written proof of this test and the results are required for school attendance.

4. CUSTODY PAPERS

(If applicable) Per State of Ohio Law, (ORC 3313.672) and the Missing Childrens Act, we are required to have an official, court stamped custody document when both natural parents are not in the home.
