Requirements

As you prepare to register your student, it is important to be aware of the procedure the District has established so that the process can go smoothly. If you are transferring from another District, your student’s records will need to be transferred to his/her new school. To register in the Hilliard City School District, please provide the following:

1. PROOF OF RESIDENCY:
Proof of residency may be a rental or purchase/closing document showing your name and address or a lease agreement that lists the parent/guardian’s name and the name of the students to be enrolled in our district.  In situations where one family is sharing a home or apartment with another, we will need for the rental agent to add the sharing family, children’s names included, to the original lease, or for the homeowner to complete an Affidavit for Shared Residence form.  (Kindergarten: We will not screen any child without proper proof of residency.)

2. BIRTH CERTIFICATE:
The Original or a Certified Copy of your child’s birth certificate.

3. IMMUNIZATION RECORD:
If you have an incomplete record or you do not have one, please contact your physician's office for a copy of this record. You MUST bring this when registering your student.

Required immunizations include:

  • Diphtheria, Tetanus, Pertussis (DTP) - 4 doses :
    If the dose was administered before the fourth birthday, a fifth dose is required.
  • Polio - 3 doses:
    If the third dose was received before the fourth birthday, a fourth dose is required.
  • Measles, Mumps, Rubella (MMR) - 2 doses:
    The first dose must have been received on or after the first birthday and the second dose at least 28 days after the first.
  • Hepatitis B - 3 doses :
    The second dose must be given 28 or more days after the first, and the third dose at least eight weeks after the second, and at least sixteen weeks after the first. To receive the third dose, the child must not be less than six months of age.
  • Kindergarten registrants beginning August 2006
  • Varicella (Chickenpox) - 1 dose: The first dose must have been received on or after the first birthday. Children having had the disease are exempt from this requirement. However, they will need a written statement from the parent/guardian or healthcare provider to fulfill this requirement.
  • Tuberculosis Test (TB) - The Mantoux Test 5 TU PPD is required if the child was born in a country other than the U.S. or has traveled out of the country for more than 90 days.

4. SOCIAL SECURITY CARD:
Your child’s Social Security Number will grant the school district permission to use the SSN as the student’s ID number for record keeping purposes, including the Ohio Education Management Information Systems.  If no Social Security number is available, the student will be assigned a number for identification purposes.

5. CUSTODY PAPERS:
If applicable. Per State of Ohio Law (ORC 3313.672) and the Missing Children’s Act, we are required to have a copy of custody documents when both natural parents are not in the home.

Incomplete registration packets will be returned to the parent.  Registration for kindergarten will not be complete, and students will not be added to class rosters, until all items are submitted and approved by the District Records Compliance Officer.
